Please note, in order to receive the scholarship experience you are applying for, you need to meet the following eligibility requirements:
Have spina bifida or be an immediate family member or caregiver for someone with spina bifida.
Live in Illinois.
Be willing to share images of your child and your family to used in marketing materials.
Have not previously received a scholarship experience in the 18 months.
Note: Participants will be selected based on meeting eligibility criteria and available funds. In the event that the number of applicants exceeds the funds available, only then will income based questions be used to determine the award of the scholarship experience applied for.
Scholarship experiences are limited to immediate family members in the same household and one additional caregiver.
Applicants will be required to submit valid proof of IL residency (such as an Illinois issued drivers license), proof of diagnosis, and an image of your child or family. Needs for documentation may be subject to change.
